IMPORTANT REMINDER for the New School Year:
 
Make sure your child is ready for 6th, 7th, and 8th grade! Students 11 years of age and older (entering 6th, 7th, and 8th grade) will be required to have one dose of Tdap vaccine and one dose of Meningococcal vaccine.
 
To make sure your child is ready for the next school year, please send proof of these vaccinations so we can update our school records. If your child has not had a Tdap vaccine (in the past 5 years) or Meningococcal vaccine, please take your child to either the Yuma County Health Department (928) 317-4559 or to your health provider to have your child immunized as soon as possible.
 
To comply with Arizona State Law (A.R.S. 36-671 and 15-871) your child will not be allowed to attend school until a record of immunization has been submitted. Local physicians and the Health Department do not forward immunization information to schools. It is the responsibility of the parent or guardian to be sure the school receives it.
